Installed the latest vslx file and the isn't showing up either in C# Add Generation Files in the edmx nor under general add files.
I'm working under German Windows but with an English VS 2010 version.
Could that be the problem (my standard installation path is "Programme" and not "Program Files"
How did you set up the installation? Is Programm Files hard coded?

I had this same problem. The extension manager shows that CslaExtension is installed, but nothing shows up under "Add new item". I found the directory where the extension was installed (on my XP machine it is at C:\Documents and Settings\<username>\Local Settings\Application Data\Microsoft\VisualStudio\10.0\Extensions). Visual Studio creates a cached representation at the subfolder Josip Budimir\CslaExtension\1.0.1\~IC. I had a look in there, and in the ItemTemplates subfolder is a zip file. The file names inside the zip file were truncated. So I extracted the item template zip file that has the .vstemplate file and shortened the file names, rezipped everything back into the vsix file and reinstalled. Now I see the item under C# items.
